Scope of Services

A Team provides functional health consultations, personalized health plans, and wellness education through an online platform. Our services focus on lifestyle, nutrition, and holistic health strategies. A Team Health and Performance may make diet recommendations & recommendations regarding the use of nutritional & herbal supplements in order to supply nutrition to support the physiological & biomechanical process of the human body. Although these foods & supplements may be suggested with a specific purpose in mind, their use is designed to support given aspects of metabolic function. The use of nutritional supplements may be safely recommended for patients already using pharmaceutical medications, but some potentially harmful interactions may occur. For this reason, it is important to keep all of your healthcare providers fully informed about all medications & nutritional supplements, herbs, or hormones you may be taking. I have elected to seek wellness optimization. I understand that in the practice of wellness optimization, some treatments are considered "alternative" by the conventional medical community & that there are some risks to treatment. I understand that some of the recommendations may not be approved by the FDA. I understand it is recommended that I see my primary care physician if I desire only FDA-approved treatments for my conditions. I understand that it is important that I provide accurate & complete health information concerning my illness, hospitalization, medications, supplements, allergies, & other pertinent items. I understand that A-Team Health and Performance is wellness care provided & the services provided are not meant to replace my primary care provider.

Eligibility

You must be 18 years or older or have parental/guardian consent to use our services. By engaging with us, you confirm that the information you provide is accurate.Termination of ServicesWe reserve the right to refuse or terminate services if a client is found to be providing false information, engaging in inappropriate behavior, or violating these terms.

Telehealth Agreement

By using our virtual services, you understand that telehealth has limitations compared to in-person visits. You acknowledge the potential risks of online communication and consent to receive health guidance via telehealth. Services Provided:

Appointments & Rescheduling Policy

Cancellations or rescheduling requests must be made at least 24 hours/days in advance.Late cancellations (within 24 hours of the appointment) will be charged a $25 cancellation fee or forfeit the session fee.

No Refunds Policy

All consultations, services, digital programs, and health plans are non-refundable once purchased.We do not offer refunds for missed appointments, dissatisfaction with services, or changes in personal circumstances.Payments for multi-session packages or memberships are non-refundable once the program has begun.

Modification of Agreement:

Any modifications to this Agreement must be made in writing and signed by both parties.

Governing Law:

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas.

Entire Agreement:

This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings of the parties.
